The problem with the XP sacrifce is that it is not even across classes. A thief could well afford to sacrifce a little XP for HP, but a fighter has to slog so hard for levels, he is less likely to see the value.

Hmmm yeah all those ideas are pretty worthy of consideration! Necromantic blood sacrifice is my favourite.
In mechanics terms, what about an "xp sacrifice" - xp is a tough one to give away, but for permanent hp's, could be worthwhile.

Is the "manual of health" a 2nd ed item? You know, one-shot book you read that pumps your con (or other stat).

Seems like an Ioun Stone type ability. Dark mauve dodecahedron. +8 max hps.

What about some sort of nasty necromantic blood sacrifice type spell?

Or a druidic ritual that links your life force with that of the earth or sumpfin.

What about the "Life-Gem". Created at the same time as the Soul-Gem. Get sucked into it (after solving gem related mysteries), then get your party mates to figure out a way to get you out and BAM all future hp rolls are maximised automatically.

Or some sort of chemically induced booster. Call it "Ice" for lack of a better name. 2d12 temporary hitpoints. Expensive, and some, you know, mild side effects.

That sounds like something better done as a customisation for the particular character, rather than a generic set of rules open to everyone.
I like the idea of each character being able to develop certain idiosyncrasies that nobody else has or necessarily even knows about.

What about the possibility of prestige classes when characters reach 'name' level? If Pan wanted to be a Spymater for instance? There is not any real scope for advancement after you hit name level. Could we build a rule about this?
